where to put uploaded files?

This might not deal directly with PEAR but I plan to use QuickForm to
upload files from a form.
I'm developing an application where users will upload files. I've
not done this before.
Each user has a unique login/ID.
Do I create an ftp directory for all files then manually create a
subdirectory for each user?
How do I ensure that only localhost and the application has access to
those files?
Ideas, code and link recommendations are MUCH appreciated!
