QuickForm: how do you allow the user to change his uploaded picture?

I have a form where the user can upload a picture that
works fine. I use the same form for editing but the
upload does not work on that - working on that. Even
when it does, it would only replace the database field
of the picture name not the actual file in the file
system.
I need a way to allow the user to delete their
existing picture AND / OR load another one.
The database just holds the name of the file.
Any ideas or links to existing articles?
