HTML_QuickForm not displaying file field

Hello Everyone, I am having a strange problem with HTML_QuickForm. I created a one page form that works fine except for the fact that it won't display any of the file upload fields. Has anyone heard of a problem like this before? If so what is the problem and how was it fixed? Also I would appreciate any input on debugging something like this. No error messsages are being thrown and everything else works fine so its a bit difficult trying to narrow this problem down.
