Could some one show me or point me to a good example of a tabbed form
with HTML_QuickForm_Controller?
This isn't at all specific to HTML_QuickForm_Controller or even to PEAR, but you could put the sections of the form you want as separate tabs in DIVs and then make them into tabs with something like this:

You could probably create/extend an HTML_QuickForm Renderer to output the HTML in this format for you. This particular solution requires Javascript to create the tabbed look, but degrades nicely when Javascript is disabled.
