First off, FormBuilder_Frontend isn't an actual PEAR package as yet,
so this itn't really the right place to ask questions.
The problem looks to be that the table's DO isn't getting created. Try
adding DB_DataObject::debugLevel(1); and see if you see an error just
before this.
