Getting Pear Package Manager for Windows

Based on the website, The Pear PkgMgr is supposed to be included with the
PHP 4.3.0RC1+ distribution.
However, it doesn't seem to be part of the default Windows binary distro for
RC3 or RC4. There is no pear.exe, pear.php or ./pear directory in either of
these distributions. Do I need to build it from source?
Also, once I get it installed, how do I configure it to log into a
firewall/proxy server in order to get to the archive at pear.php.net?
