A few ideas:
- Fixing bugs:

you can contribute patches for known bugs in pear packages.
- Adding/improving documentation. You can check the format used for
documentation (DocBook) here:
http://cvs.php.net/cvs.php/peardoc/en
- Adding planned (but not yet implemented) functionality. You can check for
TODO files (in cvs) for this, but I would suggest discussing anything you
might like to implement with the package maintainer first.
- And of course, the preferred way: developing your own package :)
