QuickForm elements not displaying

Hi, I have been having some problems with some Quickform elements not displaying. Specifically in the following code, the date and imageFile components do not appear. I have found that changing the order of the elements can affect whether they appear or not. QuickForm 2.10 Pear 1.0 PHP - 4.1.2 Apache 1.3.26 Has anyone else experienced this?
