Quickform "corruption"

For well over 2 months (and since about version 2.5) I have been
experiencing inexplicable corruptions in quickform. Everything is going
fine and then all of a sudden quickform starts throwing errors. The
messages are not always the same but this time I got:
Fatal error: Cannot redeclare class html_quickform_text in
/a/local/lib/php/HTML/QuickForm/text.php on line 33
I have also seen errors from textarea.php. The only thing I have found
that will fix this is to re-install QuickForm. I have moved files and
diffed them against the reinstalled version and they are exactly the
same. Things go fine with the new version for a few weeks and then it
happens again.
Any ideas what could be causing this? Has anyone seen this before?
