I completely uninstalled quickform, manually deleted the folders, reinstalled 3.0Beta2, changed include_path in my php.ini file to *only* include the pear directory and I still don't get the asterisk.
This is on my dev machine whereas the other problem was in my production environment. I have seen the other problem 5 or 6 times in the last 6 months and I understand how you would think that it is a problem with my environment... but the only way I can fix it when it does happen is to uninstall and re-install QuickForm. After a reinstall all is well for another 2-6 weeks. I suspect it is a scalability problem with something going wrong when the application is under a heavy load. Whether that something is my code (more likely) or QuickForm is unknwon but the end result is that QuickForm gets gummed up. Maybe it is a problem with PHP Accelerator. I will look at it more closely next time it happens...

Culley, It should work. You should make sure your installation is ok, try to clean install by removing the QuickForm directory maybe. I remember you told a few weeks ago about some other problems you had of multiple class declaration. These might be related.
