Could it be this template is not taking into account the possibility of some elements not having a matching label? Well, looks like it does not take into account the possibility of reset buttons, because of this:

I *think* it should be
{elseif $element.type eq "submit" or $element.type eq "reset"}
I am not that familiar with Smarty, so please test this or try to ask Thomas Schulz, who authored the Smarty examples.
