There appears to be a bug in the hierselect element of Quickform. When
the form is initially displayed it shows the first entry for both of the
select boxes. However, there is no guarantee that the selected options
are related. Once one 'onchange' event occurs the two boxes are
synchronised. Maybe there needs to be a Javascript initialisation
function?
