DB_DataObject_FormBuilder usage

Hello,
For the last day I have tried to use the DB_DataObject_FormBuilder class
for a high-abstraction data objects framework. No matter what I do, I
get the following warnings:

a) Where can this come from
b) Where can I find documentation other than the one I phpDoc-ed from
the class.
