Good question.
PEAR packages can be tested using PHPUnit. It becomes problematic somehow when you have things like
database connection, remote server access etc.
Do you have anything specific in mind ? There are a lot of ressources of testing on the web. The
book Pragmatic Programmers has very good chapter on it.
