Quickform Configuration Problem?

I am encountering MAJOR difficulties in getting PHP to work on my hosting
service. Here's the problem: when I try to render a simple form, nothing
happens. NOTHING. Eventually, it stops and gives me an HTTP 404 error
(cannot find server). The only element I can successfully add is "label".
Due to a proxy configuration issue, the QuickForm package was installed
manually, and is in the normal PHP include path
(/usr/local/lib/php/HTML/QuickForm.php).
Basically, I'm stuck. I'm guess something is timing out, but I really have
no idea of what.
