What is going to happen is packages will be more wary to change their status away from alpha until they are sure the API is a good one. Some may even design API extensibility into the API to allow for flexibility without breakage. Imagine that :) A PEAR with a dependable API is a usable PEAR, for both new users and enterprise users.
