Bug description: package-approval.php needs ability to reject proposals

public_html/admin/package-approval.php only has the ability to approve
packages. The power to reject packages needs to be added.
Part of this process should be modifying the add() method in
include/pear-database.php. It would be helpful to have a link for
rejecting the proposal.
