PECL/ssh2 package does not support LibSSH2 versions >= 0.16

Since Version 0.16 of LibSSH2, LIBSSH2_APINO was removed from the public header file.
It causes compile error for PECL/ssh2 package... To patch ssh2.c:
search and change following line (LINE 480 on ssh2 version 0.10):
#if LIBSSH2_APINO < 200412301450
to:
#if (defined(LIBSSH2_APINO) && LIBSSH2_APINO < 200412301450)
----
search and change following line (LINE 1216 on ssh2 version 0.10):
#if (LIBSSH2_APINO > 200503221619)
to:
#if (defined(LIBSSH2_APINO) && (LIBSSH2_APINO > 200503221619)) ||
(defined(LIBSSH2_VERSION_NUM) && LIBSSH2_VERSION_NUM >= 0x001000)
Compile again!
