First thing is having an idea and some motivation :-) for getting started, well there are different sources. first of all the PHP source and looking at other PECL modules. Many functions in there are quite trivial and learning the structure is quite useful as copying from other extensions usually is a good way to solve issues. But well, starting from source is a bit complicated, so you probably need something to read.
