I messed up the example a bit. It should read:
<? for($loop=0; $loop<=count($Name); $loop++): ?>
<? print $Name[$loop]; ?><br>
<? print $Address[$loop]; ?><br>
<? print $Phone[$loop]; ?><p>
<? endfor; ?>
