This may not be the same error condition, but I get similar problems in my environment when I've got Apache configured for NameVirtualServers with PHP4RC1 and Oracle8 support. The first connection an apache process makes to the Oracle server is fine, but subsequent connections reliably fail. Changing the Virtual Server setup over to IP based seemed to fix the problem.
