i cannot track it down right now, but obvious place is
zend_execute.c" line 718
offset = get_zval_ptr(op2, Ts, &free_op2, BP_VAR_R);
if (offset->type != IS_LONG) {
checking get_zval_ptr shows that it *can* return NULL on IS_UNSET so that
offset->type will be a ZPR.
