This happens when your Apache process does not have the PHP module
available. I have seen it happen on sites where the admins swore to me a
million times that they didn't change a thing. In the end it turned out
that they had rebooted their server and their /etc/rc.d/init.d/httpd
script started a different Apache server than the one they were running
before the reboot. To avoid problems like this, make really really sure
you only have a single httpd binary on your system and that the various
scripts that start and stop your server are all referring to the same and
correct httpd binary.
