Brian, We currently use the PDFlib that requires a license for commercial use. It shoulds like you will also need to use the FDF function in PHP to fill forms. Both links to the documentation do a good job of explaining and providing an explanation as to how to impliment these feature in PHP. The last portion of your request is not possible under the current libraries that are out there. I was unable to find one anyway.
