You have to load php_mssql.dll extension.
Then in php
$dbhost="myDBHost"; // or IP address of the server
$dbusername="username";
$dbpassword="password";
$dbname="MyDatabase";
$connection=mssql_pconnect($dbhost,$dbusername,$dbpassword);
mssql_select_db($dbname);
