I have a combination of MS SQL 7.0 (other server then web server), IIS 4 and WinNT 4.0 SP5 working. 1) Did you installed MS SQL Client software on web server? 2) Have your I_USR... user "full access" rights to directory, where php_mssql70.dll is stored? 3) Did you set up properly extension_dir to directory, where your dll's are stored (e.g. c:\winnt\system32\inetsrv\)? I did this three steps and it works fine.
