<INPUT TYPE='CHECKBOX' ....>
should change to
<INPUT TYPE='CHECKBOX' CHECKED ....>
wherever you want the checkbox to have a tick in it. Similarly
<OPTION VALUE=....>
should change to
<OPTION SELECTED VALUE=....>
if you want it to be selected.
