Which PHP survey?

http://php.resourceindex.com/Complete_Scripts/Survey_and_Voting/ I need to create a survey that has: - radio button answers - checkbox answers - matrix with radio button answers, and - free form text answers I also need to be able to give the administrator of the survey the ability to sort the answers. Has anyone ever used any of the surveys listed on the URL above and have comments, positive or negative about them? Anne
