Error Message-Access Violation at 01194D15

Hi all, I' fairly new @ php and SQL7Server. With that said, I have this problem: Im testing the connectivity of my PHP4.0.2 build with a SQL7Server.. Ive created a simple form that is designed to pass a name and password variable to a php page that inserts the values of these variables into a dbase table SQL7...It took a little while to get it to connect..but its connected...now the problem Im having is a PHP Violation...Can Anyone help me??
