Make sure that:
1. Your anonymous internet user (I_USR_etc) have execution rights in the php
directory.
2. In the Internet Service Manager you have configured the isapi filter and
the extension mapping not for the whole server but just for the website(s)
in which you want php enabled.
You can also try installing the latest service pack (6) for NT4...Good luck!
