Please try and download the .zip file again. Something must have been screwed up with that ISAPI dll. I replaced the .zip with a new build and it seems to work.

Add a new entry to the Application Mappings; Use the path the php4isapi.dll as the Executable, supply .php as the extension, leave 'Method exclusions', blank, and check the Script engine checkbox." This should at least allow you to run regular php files. Please let me know how it goes.
