My first thought would be the fact that you're not taking case sensitivity
into account.
In your HTML you specified the tag as "Contact", this would have PHP save it
in a variable called $Contact. Now in your PHP code you call upon the
variable $contact which is unique from $Contact and since currently contains
no value will draw up nothing. You probably have this same issue with your
other variables as well...syncronize the HTML and PHP code so the names are
identical and your problems will more than likely go away :)
