PDFs and DBs

Hey there, I'm wondering if anyone knows of a way to take a PDF like one of a W-9 form for example and insert some data into it from mysql. For example, I want to fill out the form electronically and possibly display it in the browser possibly. Like this: A user enters information like first name last name, social security number and hey hit a button a screen comes back the w-9 already filled out and all they have to do is print it. If we can display the pdf using the pdf plugin for Netscape or IE, then that would be totally cool. Thanks! Marky
