Will not read ini file!

For the life of me, I cannot get PHP 4.0.2 to read the ini file - at
least that is what I am learning from phpinfo() - the extension_dir is
empty.
Here is my setup:
The php4isapi.dll is in c:\php\sapi
The php.ini is in c:\php\sapi
IIS is configured to use c:\php\sapi\php4isapi.dll
The php.ini file has "extension_dir=c:\php\extension\" and I have tried
"Extension_dir=./"
When I run

echo phpinfo();
?>
the extension_dir is empty - no value.
Am I overlooking something so obvious that I cannot see it?
