I'm not sure, but it seems I've allready gave you an advice to use DHTML to create the form "on-the-fly", but let me know more about the problem. Do some determinated class of nomination have allways the same number of entries or its number is undetermined ? If the number of entries are constant you can use a "select" tag to choose the "nomination" and call the apropriate form to fill up. If not, I think the only possible solution is create the apropriate form "on-the-fly" using dhtml. I can send you an exemple on this matter (let me know the email I should send it to).
