Designing an input form for one-to-many etc.

I've been staring at this problem so long that the answer will either
come to me in a dream or after a lot of caffeine. A fresh set of eyes
and some insight or advice would be greatly appreciated.
I've finally got the time to get to work on an awards database [special
thanks to Mark Nold for his normalization advice back in July] and I'm
having problems trying to conceive the input form because of the
possibility of many multiple entries for the same nomination.
Examples:
Best Actor might have 5 nominees but Best Editing might have 3.
In the early Oscars, an actor could be nominated for a body of work, so
you could have one actor nominated for two films in the same nomination
e.g.:
Best Actor
John Doe for "PHP or Death"
Bob Smith for "Normalize This" and "Submitting Forms"
C.P. Yu for "Overheated"
A special effects category could have multiple people for each
nomination.
Harry Chest for "Inside Bill Gates"
Larry, Curly and Moe for "SQL: the Sequel"
Etc., etc.
Needless to say I'm having a hard time trying to figure out how to
design an input form that will take into affect the variations.
Any ideas?
