You should start by changing all of those lines to "application/x-httpd-php" for
conventions and sanity's sake (php4 uses "application/x-httpd-php" for php refs now).
It sounds like you didn't properly upgrade; it's possible that php3 support files are
still loading in your system.
Please find and delete all copies of the following files from your system (they come with php3
distro for win32):
php.exe
any .dll file prefixed with php3 in the name.
Then please reboot, and reinstall php4, making sure your apache config properly points at the php4
exe.
