This is already mentioned in the general install-instructions of the manual, and will be stated in
the next PHP-Version

The install.txt file for the windows binary installation mentions copying the php4ts.dll to the
%SystemRoot% for Apache and Netscape servers. It seems to be required for IIS, if you're using
the php4isapi.dll. This copy step needs to be added to the ISAPI installation process.
