This might be slightly off topic - but there is a great script written in
Miva called simplemail. It is a great backend form processor. You give it
the url for your page with a form on it, it generates a form for you to
complete for range and item completion, etc. and saves it in a database.
When someone uses the form simplemail processes it and either sends the form
or generates a custom error page.
If some of you would like to help me convert this to PHP, I think we just
might have a valuable tool.

php.net/mail -- send them an email with some words, and confirmation of
their choices.
a reference number can only be unique if you know what other reference
numbers have already been sent. for example if you store the form data in a
mysql table, then I'd have an auto-increment ID field, which will start at
1, and work it's way up with each form submission.
if you're not working with a database, then you need some other way of
remembering what the last "unique" id was, and incrementing that number by
1.
