Installation help - what did I miss?

What am I missing?
I'm trying to install PHP 4.0.1pl2 on my NT 4.0 machine (service pack 3),
running Microsoft PWS 4.0 (as a developement platform). I've read the
following documentation:
1. the readme file that came with the installation package 2.

... and I've done the following (according to README.TXT):
1. unzipped the php files into d:\program files\php3
2. copied PHP4TS.DLL to c:\winnt\system32 (MSVCRT.DLL was already there)
3. copied php.ini-optimized to c:\winnt\php.ini and:
made doc_root = "d:\project_files\www" (top of the PWS web
directory)
made extension_dir = d:\program files\php3
turned on the following extensions:
extension=php_dbase.dll
extension=php_dbm.dll
extension=php_mssql.dll
extension=php_odbc.dll
4. edited PWS-php4.reg to look like...
REGEDIT4
[HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\w3svc\parameters\Script
Map]
".php"="d:\\Program Files\\php3\\php4isapi.dll"
5. added Execute access to the top directory of PWS, using PWS Manager.

I did
the following:
6. edited my registry following the instructions under "Windows 95/98/NT
and PWS / IIS 3"; I'm only adding .php as an extension for now. (I was
somewhat surprised that the registry information wasn't included in
README.TXT. Is this a left-over from PHP3 ?)
I rebooted.
Then I got a simple working .php file from my ISP (who supports PHP3) and
put it in d:\project_files\www. I pointed a browser at this file
(http://127.0.0.1/test.php) and stepped back to watch. A connection was
made to the web server, then it just sat t here, not serving up anything
("Waiting for reply.."). I checked NT Task Manager and saw that php.exe
was running. Other pages (asp and html) were still served up nicely.
I've dug around a bit to see if there was more information about PHP
installation, but I didn't come across anything that helped. So, what did
I miss?
