IDG Books software permissions for MySQL/PHP Applications

Greetings. I am a Permissions Editor at IDG Books Worldwide, Inc. We publish the Best-selling ...For Dummies computer reference book series. In order to provide our customers with an idea of the types of products available in certain topic areas, we include demo/trial/shareware versions of commercial products on CDs bundled within our books. I am one of the persons responsible for obtaining permissions for the products we hope to include on the CD to be bundled with our new title, MySQL/PHP Applications. The author is interested in including PHP 4.x for Unix and Windows among these products. In order to include your product, we need written permission. We have a standard "boilerplate" permissions agreement that would allow this. If it is acceptable to you, please contact me via return email immediately, providing me with a contact name, address, phone number, and fax number. Please also include the full name and version of the product that you would like us to use.
