I believe this is because the PHP engine thinks that the "<?xml" is a PHP opening tag.
1. Turn off short PHP tags (short_open_tag in php.ini)
-OR-
2. Use this instead: <?php echo '<?xml version="1.0"
encoding="UTF-8"?>'; ?>
