problem w/upload in PHP 3.0.17

I have a small script to upload image files that works fine with 3.0.12
and 3.0.16 on test boxes, but craps out on a production server which we
just upgraded to 3.0.17 -- no error messages given, just a seg fault and
no joy.
The initial page looks like this:

Any ideas? Others having this problem? It does the same thing in 3.0.17
using the new is_uploaded_file and move_uploaded_file functions. Looking
at an strace of the server process, it looks like it has the segmentation
fault just before it would start writing out the temporary file.
