PHP session handling performance

What is people's experience with using PHP 4's built-in session
handling? When storing session variables in files(default) on the
server, is performance sufficient? Or do most sites choose to
store session info in a database, and is performance better using
that method?
I read the documentation on session_set_save_handler(), but there
are no examples of writing your own to read/write session info to a
database. If you've done this, please let me know how hard it
was, how you like it, etc.
