sending malformed Host header

Hi all,
I don't know if this list is right, but I'll try it.
My Apache server responds "500 - Bad request" when I'm sending him a
request for PHP-scripted page.
The log file at server contain "[error] - Client sent malformed Host
header". What's wrong ?
Specs are: WinNT 4 (SP5), Apache 3.1.12, PHP4.0.1pl2
