I haven't run into a problem just like yours... but the "header" problem is
probably related to some output being sent too early in your program...
Check all your files to be sure there's nothing between the start of the
file and the PHP block... the "<?" should be the first two bytes in a file.
Also, if you use includes, make sure the closing ?> is the last part of the
file... you should not have any spaces or end-of-lines after the closing PHP
tag.
Maybe you have an auto_prepend_file in your php settings (check php.ini or
httpd.conf)
