There really isn't enough into to answer the other stuff -- show some source
code maybe...
But you're not correctly understanding NOT NULL.
An empty string is not the same as NULL, so you can put something like this:
insert into whatever (required_field) values('')
and the database will be perfectly happy.
Only this will make it complain:
insert into whatever (required_field) values(NULL)
If you want to disallow the empty string, you'll need to do that in PHP.
