I tend to favour putting all the php dlls in the same directory (along with
php.exe if it is a cgi application). This setup works fine, and since the
php dlls are not going to be shared by any other applications, it makes
sense to me not to have them lost in the dll mire of the windows or system
directories. Kepping the dlls out of there also prevents problems caused by
old dlls lurking around in the system.

It looks like the windows readme was accidentally ommitted from the 4.0.4
zip distribution. The install file only explains what to do with your web
server after you've installed php.

I don't really know if any one person does. I think that the original was
written by Zeev, and was most recently modified by Daniel Beulhausen and
myself.
I will try and get clearer instructions included in the next release.
