The Perl-compatible regular expressions support lookahead and look behind:
$str = 'abcdefabcaadd';
echo "$str\n";
$str = preg_replace("/(?<!a)a(?!a)/", "-", $str);
echo "$str\n";
will display this:
abcdefabcaadd
-bcdef-bcaadd
