I agree that form elements can only function within the scope of a single form. But PHP does not have the same constraints. Use one form and let PHP decide on which text box to use and which database to use not the form. If you do every thing on one page it is easy. Whatever variable is passed to an If() statement will be executed by PHP. I do a lot of form and PHP and it never ceases to amass me what I can do.
